Thank you for participating in this livescan audit. You may need to contact your livescan vendor to verify some of the technical details. There is room for you to record up to 6 devices. If you have more than 6 devices AND you do not have your own store and forward please indicate this in the notes section.

If your agency has it’s own store and forward then you do not need to fill in any information on the individual devices (devices 1-6 questions below). All your information will be gathered in the store and forward section. Please be as detailed as possible.
