On September 12, 2014, at approximately 6:17 p.m., Idaho State Police investigated a two vehicle crash on I84 westbound near milepost 194, at the Hazelton exit.

Jade C. Wong, 27 of Caldwell, ID, was traveling westbound I84 near milepost 194 in a 2008 Mazda van. Wong swerved to miss a piece of tire tread on the roadway and struck a 2007 Toyota Tacoma. The Toyota rolled pinning the driver, Dale L. Dietrick, 63 of Eugene, OR. Dietrick was taken by air ambulance to Eastern Idaho Regional Medical Center in Idaho Falls. Wong and his passenger Rita Ma, 29 of Caldwell, ID, were not transported. All occupants were wearing seatbelts.

One westbound lane was blocked for approximately two and a half hours.
